| Health Forms for Summer Camp Class 1 Health Form is to be used by all Scouts attending Camp Bartle, Camp Naish, Cub Scout Day Camp, Bear Resident Camp, and Webelos Camp. This form is completed annually. Class 2 Health Form is to be used by all Scouts and Leaders, under 40 years of age, attending any Boy Scout Camp. This form is only good for 36 months, so if you are not sure if yours is still valid, contact the Campmaster. Class 3 Health Form is to be used by ALL Leaders over 40 years of age, attending Boy Scout Camp and ALL Camp Staff Members. This form is completed annually. |
